Permission to Enroll Form

This form is intended for a student that needs to make changes to their enrollment for the following reasons: Audit a course (no credits earned), Instructor waives pre-requisites, Instructor grants permissions to special course work or units earned (credits), Instructor allows students to add to closed course, and Late add after first week of classes.

The document pictured below is a two-sided document to be filled out by the student and returned to the Student Solution Center for processing. The original form is kept at the campus at the Student Affairs Office.

Within the Student Solution Centers, Advisors and Information Specialists can process this form through PRISM for the following:

  • Adding/Dropping (Late) a course after the first week of classes
  • Pre-requisites override (Placement Testing, Retakes, or with permission from instructor)
  • Instructor Permissions Required (Special courses, lessons, ect.)
  • Override Enrollment Limit (Number of students allowed in the course via PRISM)
  • Time Conflicts (Student has two courses that conflict/offered at the same time)
  • Credit Overload (Student enrollment over 18 credits)
The UW Colleges Admissions and Registrar's office processes this form for the following reasons: 
  • Auditors (students enrolled into courses without receiving credit)
  • Unit Changes (students taking a course for more than designated unit load, increasing credits)
